Care Coordination for Inmate Health Services

Chronic Disease And Special Needs Management

Chronic diseases—such as diabetes, heart/stroke, and congestive heart failure, require special care and management. Patients with chronic diseases are the highest utilizers of correctional healthcare time and dollars. Correctional Health Partners, along with our clients, develops clinical standards and procedures, educational materials, and tracking mechanisms for chronic conditions to ensure the patient receives the right care at the right time to control their disease. Through these innovative strategies, we:

  • Facilitate planned and proactive management of care
  • Standardize and systematize disease interventions
  • Cross-reference evidence-based standards to develop “best practices” for disease and chronic care management
  • Proactive treatment plans tailored specifically to individual patients who require close medical supervision or multidisciplinary care
