Prison Health Services Care Coordination
Correctional Health Partners’ Intensive Facility Utilization Review staff works within the hospital, or in post-acute care settings, to ensure accurate and timely offender healthcare, and to determine if hospital treatment is necessary. Our Care Management team also reviews evidence of quality of care concerns and variation in inpatient utilization (such as delays in care).
The CHP Care Management team is comprised of registered nurses who are supported by our Medical Director and the Manager of Clinical Operations. This team also works closely with our Prior Authorization team to track all inpatient stays.
A CHP Care Manager helps to coordinate care while the patient is in the hospital by:
- Monitoring patient daily progress
- Communicating with the hospital physician to assure the patient has the right care at the right time
- Facilitating authorizations for the services needed when leaving the hospital
- Coordinating the discharge plans between the hospital and the infirmary staff to make sure the offender’s return is safe and follow-up care is effective
